Schedule: Full Time Shift: Monday-Friday Location: 765 Image Way, Orange City, FL 32763 The role you’ll contribute: The Executive Director of Population Health provides leadership, strategic direction, and operational execution to the AdventHealth Physician Network (AHPN). Population Health is defined as "A model that delivers high quality, people-centered care through highly efficient and effective processes at every venue of care throughout a highly aligned, meaningfully connected ecosystem throughout a patient's lifetime." Under the supervision of the SVP of Population Health, the Executive Director coaches, mentors, and leads Directors, Managers, and staff in the daily operations of assigned areas. In partnership with the AHPN and other Population Health teams, this role engages with stakeholders in planning, design, execution, and support of Provider Engagement & Performance, Quality & Network Design, Chronic Care Management, Network Operations, and Network Development, ensuring effective support of AdventHealth policies. This position is responsible for implementing structures, protocols, and processes to achieve the strategic vision for performance in the network, operational delivery of established protocols and processes that drive network performance in AHPN value-based arrangements, including lowering total cost of care below contracted targets and exceeding quality and experience requirements. This leadership role drives performance in quality, utilization, and service delivery expectations, including connected, convenient, and complete care. The Executive Director is responsible for supporting the organization’s Population Health employed and community providers through contracting and strategic alignment, market engagement, network adequacy, and overall network integrity for value-based Direct to Employer, Commercial, and Government contracts. Accountable for creating, deploying, and operationalizing protocols and systems such as analytics, care management, interoperability, communications, education, and CDI. Adheres to the AdventHealth Corporate Compliance Plans and all applicable regulations. Actively participates in outstanding customer service and maintains respectful relationships with all. Executes the maturation of the AdventHealth Physician Network (AHPN) performance, operations, and culture to be the first choice for services in its market. Directly responsible for all elements of the Population Health team, including clinical, operational, and strategic leadership, with the goal of driving value in network contracts. Accountable for network performance, provider engagement, and performance responsibilities within each market, as well as program health management and community care. Qualifications The expertise and experience you’ll need to succeed: Master's degree in Business or clinically relevant field (MBA, MSN, MHA, etc.)
