About the Role

The Company is seeking an Executive Director of Population Health to provide leadership, strategic direction, and operational execution for its physician network. The successful candidate will be responsible for driving a people-centered care delivery model that improves the quality of life for individuals and groups. This role involves coaching, mentoring, and leading a team to ensure the delivery of high-quality, efficient, and effective care across a seamless continuum. The Executive Director will also be accountable for the network's performance, including individual provider engagement, and will play a key role in the organization's population health strategy, with a focus on value-based care and the integration of community and employed providers.

Applicants for the Executive Director position must have a Master's degree in a business or clinically relevant field and a minimum of 5 years of related experience. A population health certification program is preferred. The role requires a leader who can drive the performance of the network in value-based arrangements, with a focus on lowering the total cost of care and exceeding quality and experience requirements. The ideal candidate will have a strong background in population health management, including chronic care management, network operations, and network development. They should be adept at engaging with stakeholders, ensuring compliance with policies, and leading a team to meet the organization's strategic vision for performance. The Executive Director will also be responsible for the implementation of structures, protocols, and processes to support the network's performance and must be committed to the values of the organization, including a focus on wholistic, efficient, and effective care.
