About the Role

The Company is seeking an Executive Director of Population Health to provide strategic leadership and operational execution in the delivery of high-quality, people-centered care. The successful candidate will be responsible for the maturation of the population health model, ensuring the seamless and effective care processes that are aligned with the organization's mission. This role involves coaching and leading a team to engage with stakeholders in the planning and support of various population health initiatives, including provider engagement, performance quality, and network design. The Executive Director will also be accountable for the implementation of structures and processes that drive the performance of the network in value-based arrangements, with a focus on lowering the total cost of care and exceeding quality and experience requirements.

Applicants for the Executive Director position must have a Master's degree in a business or clinically relevant field and a minimum of 5 years of related experience. A background in population health management and a certification in a relevant program are preferred. The role requires a leader who can drive the organization's population health strategy, with a strong emphasis on market engagement, network integrity, and the development of protocols and systems for analytics, care management, and interoperability. The ideal candidate will be adept at fostering a culture of performance and be dedicated to the values of the organization, including a commitment to customer service and compliance with all regulatory standards.
