DIRECTOR OF NURSING - Skilled Nursing and Assisted Living
Major Duties and Responsibilities
Plan, develop, organize, implement, evaluate, and direct the Nursing Department as well as its programs and activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Develop, maintain, and periodically update the Nursing Department’s Policy and Procedure Manual and nursing objectives and philosophies.
Assist the Administrator in developing and implementing the appropriate plans of action to correct identified problem areas.
Provide feedback to the Administrator regarding resident care and operational issues.
Assume responsibility and authority of operations when Administrator is not present.
Provide performance reviews and ensures compliance of all facility policies/procedures of all nursing staff.
Establish a system to monitor, investigate and prevent accident/incident occurrences.
Promote, support and participate in Continuous Quality Improvement/Performance Improvement Programs, including participation in appropriate committee and sub-committee meetings and individual reviews as required.
Participate in survey (inspections) made by the authorized government agencies.
Assist the Pharmacy representative in developing, maintaining, implementing and periodically up-dating written policies and procedures for the administration, storage, and control of medications and supplies.
Perform administrative duties such as completing medical forms, reports, evaluation, studies, charting, etc. as necessary.
Teach and lead staff to perform their job duties within federal, state, industry-wide guidelines and practices as well as established safety regulations in the use of equipment/supplies.
Assist in the recruitment, selection, and dismissal of nursing personnel.
Staff a sufficient number of licensed nurses and nursing assistants for each shift to ensure that quality of care is maintained.
Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times.
Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status in accordance with company policies.
Develop, implement t and maintain an effective orientation program that orients the new employee to the facility, its policies and procedures, and to his/her job description.
Conduct mandatory in-services for staff ensuring that all staff attend and receive the appropriate in-services via state and federal regulations.
Ensure that all nursing personnel are knowledgeable of the residents’ rights.
Direct monthly Nursing Department meetings.
Make daily rounds of the Nursing Department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards.
Develop, implement t and maintain a resident admission and discharge policy for the facility.
Assist in interviewing and selecting residents for admission to the facility.
Ensure that nursing staff are prepared and rooms are ready for the new residents.
Ensure all PAASSRR screen, level of care, transfer forms, etc. are complete at the time of admission.
Review nurses’ notes/audit resident charts regularly to ensure that they are objective and descriptive of the nursing care being provided and that they reflect the residents’ response to the care.
Schedule, attend and participate in Care Plan meetings as needed.
Complete census information and monitor all charting of nurses for compliance with state and federal regulations (assessments and plan of care).
Authorize restraints only when all other options have been investigated and monitor the follow-up to reduce restraints on a regular basis.
Monitor floors for completion of proper turning/positioning, bathing, call lights, etc.
Assure that a stock level of medications, medical supplies, equipment, etc. is maintained on the premises at all times adequately to meet the needs of the residents.
Develop, implement, and maintain a program for monitoring communicable and/or infectious diseases among the residents and personnel.
Develop, maintain, and periodically update infection control precautions and aseptic techniques that successfully correct problem areas.
Review and analyze infectious diseases laboratory reports and consult with the Administrator in developing a care plan for assuring that aseptic techniques are implemented.
Monitor infection control safety issues including equipment and supply storage.
Make rounds of the nursing unit(s) for the purpose of case findings, review of environmental sanitation procedures and supervision of isolation precautions/practices/standard precautions.
Ensure that nursing personnel follow established infection control procedures when isolation precautions become necessary.
Consult with housekeeping on infection control procedures for the handling of clean and soiled laundry/linen.
Monitor medication passes and treatments to assure that appropriate hand washing policies are being followed in the handling and administration of drugs, medications, and treatments.
Coordinate and oversee annual staff and resident vaccination programs.
Assist the Clinical Nurse Manager in the completion of MDS reporting in a timely fashion.
Coordinate and implement the individual therapy needs for each resident outlined by the physician’s orders and resident care plan.
Confer with interdisciplinary team individually and in conference to exchange, discuss, and evaluate patient information for planning, modifying and coordinating restorative programs and implementation of the residents’ care plan.
Encourage attending physicians to record and sign progress notes, physician’s orders, etc. on a timely basis and in accordance with current regulations.
Develop methods of coordination of nursing services with other resident services to ensure the continuity of residents’ total regimen of care.
Maintain records of resident incident reports and analyze data to identify areas of needed correction and re-education.
Review all complaints and grievances made by the residents and make a written/oral report to the Administrator indicating what action(s) were taken to resolve the complaints or grievances.
Ensure that all nursing care is provided in privacy and that nursing personnel knocks before entering a resident’s room.
Ensure that all resident care rooms, treatment areas, etc. are maintained in a clean, safe, and sanitary manner.
Maintain confidentiality of all resident care information according to facility policy and HIPAA guidelines.
Assure that all residents receive care in a manner and in an environment that maintains or enhances their quality of life and care.
Assure that each resident received the necessary nursing, medical, and psycho-social services to attain and maintain the highest possible mental and physical functional status.
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility.
Maintain a good rapport and act as liaison with families and residents.
Attend and participate in continuing education programs designed to keep you abreast of change in the profession as well as to maintain licensure on current status.
Attend required in-services regarding various topics and Corporate Compliance.
QUALIFICATIONS/COMPETENCIES:
EDUCATION
Must possess, as a minimum a Nursing Degree from an accredited school of nursing.
BSN Preferred.
LICENSURE
Unencumbered Registered Nurse License (RN), preferred requirements include Certified Director of Nursing and Wound Care Credentials.
EXPERIENCE
Must have, as a minimum, three (3) years experience in a supervisory capacity in a long-term care facility or other related health care facility.
Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices.
Must have in-depth knowledge of state and federal nursing home regulations and nursing practices and procedures.
Must possess outstanding leadership abilities and communication skills.
