Job Summary:
Oversee and manage the coordination and scheduling of home care services. The Director of Coordination plays a crucial role in ensuring efficient and effective service delivery, managing client care schedules, and coordinating with clients, field staff, and care providers. The DOC supervises and is the team leader of, the entire department of coordination.

Responsibilities:

  • Coordinate Client Care:

    • Manage the coordination and scheduling of home care services for clients.

    • Ensure appropriate staffing levels to meet client demands and maintain high-quality care.

    • Monitor and evaluate the delivery of care to ensure compliance with regulations and quality standards.

  • Staff Management and Communication:

    • Interview, train, and supervise coordination staff members.

    • Provide guidance and support to coordination staff, ensuring smooth operations and team coherence.

    • Foster effective communication channels between care providers, clients, and internal teams.

    • Collaborate with HR and upper management in matters related to staffing, performance management, and training.

  • Process Improvement:

    • Identify and implement opportunities for process improvement to enhance coordination efficiency.

    • Utilize technology and software tools to streamline scheduling and communication processes.

  • Compliance and Documentation:

    • Ensure compliance with all applicable regulations, policies, and procedures.

    • Maintain accurate and up-to-date client records, documentation, and reports.

    • Assist in audits, inspections, and quality assurance activities as needed.

Qualifications:

  • Bachelor’s degree in healthcare administration, business management, or a related field (preferred).

  • Minimum of 2 years of experience in a leadership role within a LHCSA or similar healthcare setting (preferred).

  • Strong knowledge of home care regulations, policies, and best practices.

  • Excellent organizational, communication, and interpersonal skills.

  • Proficiency in using scheduling and coordination software.

  • Ability to work collaboratively with a diverse team and manage multiple priorities.

  • Demonstrates strong leadership and problem-solving skills.

  • Personifies an excellent team leader, communicator, and is well-liked and respected.
