A growing healthcare organization is seeking a Wound Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide services across multiple skilled nursing facilities in the LaGrange, GA area.

Responsibilities:

• Manage and treat all wound care needs within assigned facilities, including cleaning and dressing wounds

• Complete admission documentation as required

• Provide guidance and support to residents and their families

• Monitor residents’ conditions and administer medications and treatments

• Document care services accurately in medical and departmental records

• Adhere to infection control policies to ensure the safety of residents and staff

• Address resident needs through collaboration with a multidisciplinary team

• Maintain high standards of care in alignment with organizational protocols

• Ensure confidentiality of resident information at all times

• Track infection control data and outcomes

• Prepare and submit regular reports, including monthly summaries

Qualifications:

• Active NP or PA license in the state of practice

• Experience in long-term care preferred

• Wound care and ostomy certification required

• Strong computer skills, including proficiency in Microsoft Office

• Excellent communication and interpersonal skills

• Team-oriented with a positive and professional demeanor
