StartDate: ASAP Available Shifts: 8 D Pay Rate: $2136.00 - $2268.00

13 wks Schedule of assignment hours: M-F 8-5 + on-call 1 weeknight/wk Will there by any weekend rotation: Yes – 1 weekend per month What Region/POD are you in: GA Pod, Newnan, GA What territory will this contractor be asked to cover: Sharpsburg, Peachtree City, Fayetteville, Jonesboro, Riverdale areas What is creating the need for this contract request: workload What location and address will the contractor need to report to for their assignment: 1825 HWY 34 E Ste 2200 Newnan, GA 30265 Approx how many miles a week will the contractor travel: between 300-400 What are the weekly units of expected productivity for this contractor: 20-25 units of productivity in a 40 hr work wk What EMR will the contractor be using: HCHB Do you prefer the candidate to have home health, hospice or both experience: Must have Hospice experience and comfortable with on-call and admissions.
