SUMMARY:
Under the general supervision of the Administrative Director this position provides
quality, professional, nursing care and medical oversight to Skyridge’s Assisted Living
Elders. The Skyridge Assistant Health Director works cooperatively and respectfully with
elders, coworkers, families, medical staff, visitors, and volunteers to create a positive
environment. Responsibilities include, but are not limited to; personal care, medication
assistance, housekeeping, food preparation and service, activities, record keeping,
responsible financial management, and oversight of Elder Sharaths.
This position requires the ability to function in any designated area (housekeeping, direct
care, dining services) independently in performing duties and be able to solve problems
using good judgement. Must possess the ability to respond to the Elder’s needs in a
timely, professional manner, recognizing the elder’s rights.
ESSENTIAL JOB FUNCTION: Nursing
Oversees the care of all elders in assigned areas.
Collaboratively conducts assessments and develops care plans
Works closely with physicians to determine and initiate appropriate care
Responds to and leads emergency situations
Completes medical records on elders and performs record keeping according to
policies developed by St. John’s United in accordance with State and Federal
Regulations
Observing elders, documenting and reporting changes in elder’s condition, and
taking necessary action in a timely manner
Assesses WyndStone Elders for determination of Assisted Living Category A or
B, or C in collaboration with Health Director
Assist’s C.N.A’s when necessary
Responsible for faxing information to resident’s primary care provider and
pharmacy, coordination of physician appointments, transportation, and
communication with family
ESSENTIAL JOB FUNCTION: Leadership
Supervises C.N.A’s in assigned areas
Coach, educate, evaluate and discipline assigned employees
Assists administrator/director formulate performance reviews for peers
Develop referral and support relationships with Elders of SkyRidge
Facilitate Service Plan Meetings and other family meetings
Works closely with local hospital Case Managers on appropriate discharge
planning for current Elders in collaboration with Administrative Director
Coordination of appropriateness of Admissions, Discharges, and/or Transfers with
Administrative Director
Assists with Tours and other marketing opportunities to families and community
Facilitates appropriate staffing levels, and schedule
Manages and processes development and Quality Improvement
Rotation in on call schedule between Mission United communities
ESSENTIAL JOB FUNCTION: Communication
Attends and contributes to mandatory staff meetings, and daily shift change
report
Maintains the confidentiality of all Elder care information according to the St.
John’s Policy on confidentially and Elders Rights
Properly documents medication omissions, errors and decline of medications and
verbally passes this information on to Administrative Director.
Counts narcotics with appropriate team members and notifies supervisor with any
discrepancies before leaving the building.
Keeps the Administrative Director informed of any changes in the elder’s
condition through incidental and formal reports
Works as a productive, supportive and communicative team member
Responsible for documentation, faxing and filing of elder’s insurance paperwork
Encourage, participate and support meaningful activities for elders throughout the
day.
Responsible for properly referring elder to services needed ie: Hospice, physical
therapy, occupational therapy, speech therapy, Home Health etc
ESSENTIAL JOB FUNCTION: Direct Care
Oversight and auditing of Elder Sharath’s daily tasks include: bathing, dressing,
grooming, and related personal care as designated in the residents care plan.
Responsible for passing and reconciliation of medications to elders per provider’s
orders, following the “5 Rights” and med pass “rules” and supervision of Elder
Sharaths in their role of assisting elders in the self-administration of medications
Responsible for observing elder’s well-being and reporting changes in behavior,
and change in vital signs etc. to provider and Administrative Director
Assists with the care of the dying elder, maintaining hope, dignity and love for the
elder
Implements infection control measures and follows Colors of Hypervigilance as
needed
If designated by the Administrative Director a responsibility MAY include
driving the elders via bus or other vehicle
Facilitate a collaborative relationship between nutrition services and the nursing
department to promote quality assurance for service, temp, presentation, and
satisfaction for meals
ESSENTIAL JOB FUNCTION: Safety
Obeys safety rules of St. John’s United
Identifies, reports, and/or corrects obvious safety hazards
Attends Mandatory safety in-services
Utilizes proper body mechanics when transferring Elders
Follows established policies and procedures regarding exposure to blood/body
fluids
Wears and/or uses safety equipment and supplies (i.e. gait belts, mechanical lifts,
personal; protective equipment)

ESSENTIAL JOB FUNCTION: Employee Responsibilities
Follows policy and procedures on TB testing.
Follows policy and procedures on annual influenza vaccinations.
Attends all mandatory in-services
Attends and participates in scheduled training and educational classes to maintain
license in accord with the State of Montana
Performs other duties as assigned
Requirements REQUIREMENTS:
Minimum Requirements: Must be Licensed Nurse in the State of Montana. Have
knowledge or become familiar with Assisted Living Regulations. 3-5 years nursing
experience. Previous supervisory experience desired.
Knowledge, skills, and abilities required:
? Knowledge and skills acquired through completion of a state certified Nurse
Program
? Skills in observation of human behaviors or changes of condition
? Ability to understand written and verbal communication. Ability to interact
effectively and professionally with Elders, family members, and coworkers
? Ability to work independently and in a team
? Ability to lift and manipulate heavy objects
? Ability to understand and follow safety policies and procedures
? Ability to use various health care supplies and equipment, including blood
pressure devices, thermometers, wheelchairs, lifts, and oxygen suppliers, etc.
? Ability to understand and to contribute to Elder charts and care plans
? Ability to relate to and work with the ill, elderly, disabled, emotionally upset,
and at times, combative people within the facility
? Ability to respond to Elder needs in a timely and professional manner,
recognizing the Elder’s rights
? Ability to deal with stressful situations (i.e. Death and Dying)
PHYSICAL DEMANDS:
? Requires moderate physical work more than 70% of the time, including lifting
up to 10 pounds regularly and up to 25 pounds occasionally, standing,
pushing, pulling, lifting, walking, and bending for prolonged periods of time.
WORKING CONDITIONS:
? Work involves some exposure to dirt, dust, fumes, unpleasant odors, and/or
loud noises.
? Work involves some exposure to hazards, blood borne pathogens, or physical
risks, which require following basic safety precautions.
