Sangre de Cristo Community Care is dedicated to compassionate care and innovative programming for patients and families across southern Colorado. We are seeking a visionary leader to champion two cornerstone programs ,our bereavement and volunteer services ,while spearheading the launch of our new Dementia Care Program, part of CMS's GUIDE Model to support people living with dementia and their caregivers.
The Opportunity
We're looking for a rare leader, equal parts compassionate clinician, skilled program manager and innovative builder. This role offers the chance to honor tradition while creating something entirely new, helping us expand our reach and impact for some of our most vulnerable communities.
Key Responsibilities
Bereavement Services : Lead our grief support programming, ensuring compassionate, evidence-based care for individuals and families.
Volunteer Program Oversight : Recruit, train, and inspire a network of volunteers, creating a culture of service and engagement.
Dementia Care Program Leadership : Design, implement, and grow our GUIDE Model initiative, collaborating with internal teams, external partners and community stakeholders.
Program Development & Expansion : Evaluate needs, develop resources, track outcomes and adjust strategies for ongoing growth.
Leadership & Collaboration : Inspire cross-functional teams, mentor staff and volunteers and champion our mission in the community.
What We Offer: Competitive salary - starting at $78,478 annually
Mileage reimbursement: 62.5ยข per mile

Requirements Master's in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) strongly preferred.
Demonstrated leadership in program management and development, ideally in healthcare, hospice or aging services.
Strong understanding of dementia care, caregiver support and bereavement best practices.
Excellent communication, collaboration and strategic thinking skills.
Passion for building programs from the ground up and scaling them for community impact.
