Title: Primary Care Physician (Internal Medicine / Family Medicine)
Location: Fresno, CA, US
Job Type: Full Time | Day Shift

Key Responsibilities:

• Diagnose and manage acute and chronic medical conditions
• Perform Annual Wellness Visits (AWVs) and routine check-ups
• Develop and implement preventative care plans
• Coordinate and manage specialist referrals
• Maintain accurate and compliant documentation, including quality metrics (e.g., HEDIS)
• Provide patient-centered care with a focus on long-term health outcomes
• Build and maintain strong patient relationships within a concierge care model

• Strong clinical assessment and diagnostic skills
• Preventative care and wellness management expertise
• Proficiency in conducting Annual Wellness Visits (AWVs)
• Knowledge of HEDIS measures and quality reporting standards
• Care coordination and referral management skills
• Excellent patient communication and relationship-building abilities
• Strong documentation and EMR/EHR proficiency
• Ability to manage geriatric/senior patient populations
• Critical thinking and clinical decision-making skills
• Time management and ability to work in a personalized, concierge care setting

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
• Active and unrestricted medical license in the state of practice (California preferred, if applicable)
• Experience in primary care, preventative medicine, or wellness-focused practice

• Background in Family Medicine or Internal Medicine
• Experience working with managed care populations, especially seniors
• Knowledge of quality documentation and HCC coding
