Responsibilities
Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
Provides accurate, adequate, and timely drug information to the professional staff and patients.
Interprets medication orders (Verbal/written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles.
Prepares and dispenses medications, including sterile preparations, extemporaneous compounds, parenteral nutrition preparations, and possibly chemotherapy safely and accurately using appropriate techniques.
Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy staff
Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area.
Ordering of pharmaceutical supplies
Manage Pharmaceuticals/NDC codes in electronic health record (EHR)
Medication Administration System cabinet supply management
Accountability for anesthesia carts
340b program management
Patient education regarding medications
Review and reconciliation of pharmaceutical charges
HIPAA compliance
Attend P&T (Pharmacy & Therapeutics) meeting every other month
Active member of Swingbed Committe and monthly Peer Review Committee
Qualifications
Graduate of an ACPE-Accredited School of Pharmacy. Successfully passed NAPLEX and Kansas MPJE examinations. BS Pharmacy Degree or Doctorate of Pharmacy degree. Licensed with the Kansas State Board of Pharmacy. 340b experience preferred.
