Position Overview: As a crucial step towards becoming a Senior Medical Director, the Medical Director Associate will apply their leadership experience to quickly master the ChenMed clinical model. This role includes parallel training opportunities such as PCP Black Belt Training and Medical Director Essentials to enhance your growth and effectiveness. This outcomes-focused position includes oversight of key metrics such as patient outcomes, experience ratings, clinical gap closures, and medical cost efficiency. You will partner with your care team and leadership to achieve these goals while nurturing a positive work culture centered on love, accountability, and passion. Your Responsibilities: Accountability: Ensure strong clinical outcomes and effective care for a panel of at least 300 risk-adjusted patients. Health Coaching: Collaborate with patients to set and achieve health goals, fostering a supportive environment. Simplification: Clarify processes to facilitate actionable changes for both patients and your care team. Leadership: Conduct weekly care meetings (Super Huddle and Transforming Care Meeting) and support operational management. You will work independently and collaboratively with other healthcare professionals, participating in clinical meetings and initiatives that promote patient health and align with our organizational goals.
