Summary/Objective: Responsible for the direction and administration of the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payor program requirements. Ensures assessments accurately reflect the physical, mental, and psychosocial status of each resident; ensures accuracy of RAI to reflect optimal reimbursement for services provided; and ensures appropriate documentation to report and support services provided and assessment accuracy.

Oversees and coordinates the development and completion of the resident assessment (MDS) in accordance with current Federal and state regulations. Leads the development of comprehensive resident assessments and care plans as chairperson of the interdisciplinary team. Ensures the validity of all Minimum Data Sets before electronic submission. Ensures timely electronic submission of all MDS. Conducts monthly reviews of a sample of MDS to ensure accuracy and consistency with medical records. Coordinates the completion of each resident’s MDS in compliance with all State and Federal requirements, including calculating triggers, developing assessment protocols, and ensuring timely and accurate documentation. Develops and maintains MDS and clinical records according to regulations and policies. Participates in interdisciplinary care team meetings and stays current with regulatory changes. Ensures compliance with laws and regulations, and participates in training and performance improvement initiatives. Required Education and Experience Registered Nurse (RN) licensed in good standing. Experience or education in geriatrics and PPS assessment, with demonstrated competence in OBRA compliance. Excellent clinical assessment, organizational, and decision-making skills. Knowledge of state and federal regulations governing long-term care. Proficiency in Microsoft Office and familiarity with MDS software. Preferred Education and Experience Minimum of 2 years of experience, preferably in a fast-paced or post-acute setting. Skills, Knowledge, and Abilities: Strong communication, organizational, and project management skills. Ability to work effectively with all staff levels, residents, families, and community members. Working knowledge of the MDS and RAI process, with an interest in reimbursement systems. Basic computer skills with an aptitude for learning company-specific applications.
