Overview MDS Director – Full-Time 1st Shift at North Shore Health. Lake Country Health Services is a leading provider of skilled nursing and rehabilitation services and is part of North Shore Healthcare, with 70 skilled nursing and assisted living centers located in the Upper Midwest. We are seeking an MDS Director to lead the Resident Assessment Instrument (RAI) process to ensure accurate and timely resident assessments and appropriate documentation to support services and reimbursement. Responsibilities Direct and coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and state regulations. Lead the development of comprehensive resident assessment and care plans for each resident as chairperson of the center Interdisciplinary Plan of Care team. Ensure the face validity of all MDS before electronic submission and ensure timely electronic submission of all MDS. Conduct monthly random reviews of MDS to ensure accuracy and internal consistency with the medical record. Coordinate completion of each resident’s MDS in compliance with state and federal requirements. Calculate triggers and develop resident assessment protocols for initiation of care plans. Identify each resident’s level of functioning and ensure MDS accurately reflects reimbursement opportunities while complying with laws. Ensure CNAs, nurses and other disciplines complete required clinical documentation timely. Coordinate and assist with weekly interdisciplinary care conferences and Medicare meetings; make referrals as needed to nursing and therapy departments. Maintain MDS and supporting clinical records per regulations and center policies; review hospital records to accurately complete MDS scoring. Train nursing staff on timely and accurate clinical documentation. Support Medicare/Medicaid billing processes, generate MDS audit reports, and ensure accuracy of billing and reporting (CASPER, Validation Reports, etc.). Stays current with state and federal regulatory changes; participate in in-services and PI committees as scheduled. Adhere to center policies, HIPAA requirements, safety protocols and incident reporting; participate in the Center’s Compliance Program. Qualifications Registered Nurse (RN) in good standing with the respective state licensing board. Clinical experience, education, or specialty skills in geriatrics; demonstrated competence in PPS assessment and OBRA compliance. Excellent clinical assessment, organization, and independent decision-making skills; strong interpersonal abilities. Knowledge of state and federal regulations governing center practices; proficiency with Microsoft Office; experience with MDS software and Care Tracker. Ability to read, write, speak, and understand English; ability to work with all levels of staff and residents/families. Preferred Education And Experience 2+ years of previous MDS director or similar experience in a post-acute care setting. Experience in a fast-paced environment; post-acute care or healthcare experience preferred.
