PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Program Description:
PHS administers WholeYouNYC, a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WholeYouNYC brings together over 200 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings. Public Health Solutions (PHS) and our WholeYouNYC network were awarded the role of regional Social Care Network to support the more than 2 Million Medicaid members residing in Brooklyn, Manhattan, and Queens.
Position Summary:
The Managing Director will direct the day-to-day operations of three foundational pillars of the WholeYouNYC Social Care Network: community engagement, social care navigation, and service delivery. The MD is responsible for ensuring effective outreach and engagement of Medicaid Members, an efficient social care navigation workforce, a high-performing, comprehensive network of community-based organization service providers, and excellence in service across the board. The MD also works to build and improve the necessary technology infrastructure that supports optimal performance. The MD must be a master collaborator, internally and externally, working alongside peers to integrate network activity into PHS’ operations teams, ensure the network is effectively integrated with healthcare system partners, provide capacity-building, training and technical assistance that is responsive to network needs, evaluate impact and improve quality, and set the network up to achieve diverse sources of revenue for ongoing sustainability.
Specifically, the Managing Director will:
Accelerate performance of the network, successfully advancing it from early-stage development and implementation to full scale operations and reach.
Oversee and direct the WholeYouNYC social care navigation team, community engagement team and the service delivery network, driving performance that measurably advances health equity and addresses the unmet, health-related social needs of New Yorkers.
Create an extraordinary, high performing and diverse team that can flexibly adapt to the dynamic nature of the work.
Build and strengthen WholeYouNYC’s service delivery network, ensuring sufficient network adequacy, access, quality and service-delivery capacity.
Ensure strong performance, productivity, and quality of network activity (screening, navigation, services), prioritizing member and partner experience.
Produce and present high quality and impactful reports and presentations for funders, internal teams, and external stakeholders.
Incorporate member experience and community engagement across development, design, service delivery, quality improvement and evaluation.
Coordinate and facilitate meaningful and impactful stakeholder engagement meetings, including the Social Care Network coalition, which includes all social care network lead entities across New York State.
Together with the Senior Director of Implementation & Administration, we effectively mobilize internal capacity and streamline critical processes necessary for operational success (Finance, HR, IT, Legal, Development & Marketing).
Work with the IT and quality and evaluation teams to create and iterate the ideal technology ecosystem to meet the requirements of a Social Care Network while facilitating communication across sectors and collaborative, accountable care.
Work with legal team to advance critical compliance capacities for partnership with the highly regulated healthcare sector.
Collaborate effectively with PHS’ Neighborhood Health Services to integrate WholeYouNYC network activities where there is value.
Perform other duties as assigned.
Qualifications and Experience:
MBA or other Masters’ Degree preferred.
At least ten(10) years’ experience leading collaborative work.
Ability to thrive in a work environment that is respectful, goal-oriented, focused, and supportive of work-life balance.
