Job Description
The
Regional Director of In-Home Services
oversees the administration of the home care agency for a designated geographic region.
Key responsibilities include insuring compliance with regulatory (federal, state and local) requirements, management of staff and contracted services, and overall agency operations.
This individual is also responsible for management of services provided to participants outside of the clinic setting (e.g. domiciliary care services, durable medical equipment and appointment scheduling).
The Regional Director of Home Care Services participates with the management team in clinical oversight of QAPI programs, compliance and survey processes, and relates planning and reporting.
Assure that an adequate quantity and quality of staff are available and scheduled according to the needs of the participant. Oversees the hiring, orientation, competency evaluation, and training of all Home Care staff ensuring highly competent and qualified staff at all levels of the agency.
Performs chart audits on a timely and concurrent basis to ensure that all documentation are in place, content is appropriate for quality case management, regulatory requirements are met. Maintains written results of chart audits and educates staff to expectations of appropriate documentation.
Coordinate with the Director of Quality Improvement in preparing for site surveys by state, local and federal agencies.
Facilitates as needed conference calls and meetings with regulators.
Oversees the provision of care in all housing units to participants to promote high quality care in compliance with standards of practice and related regulations.
Recommends and coordinates training and development programs for staff.
Evaluates staffing patterns and promotes high quality hiring practices.
Monitors outcome data related to utilization of participants in NC housing and reports to QAPI.
Participates in clinical oversight of professional staff ensuring quality processes, education of staff, and documentation.
Oversees and analyzes data and outcomes including services utilization, participant and staff satisfaction, and survey results.
Educates and supports staff in collaboration with the Director of QAPI and Education as the Center and Clinical Directors.
Collaborates with physicians, consultants, community agencies, and institutions to improve quality of services and identify opportunities for program growth.
Oversees the Level 2 reporting and investigations to ensure timely and effective resolutions.
Maintains the nursing on-call schedule and participates in on-call as needed to maintain coverage and support staff.
Serves in Quality Improvement program collaborating with the Medical Director and Director of Quality Improvement.
Coordinates with interdisciplinary team to identify areas of review, ensure efficient gathering of data, submit reports, and follow up on resulting action plans.
Ensures compliance with all standards of care for federal, state and local home care regulations.
Coordinates agency committees according to regulations and as indicated.
Participates in all regulatory surveys, communications with regulators, and plans of correction.
Actively implements all Performance Improvement plans.
Coordinates with staff in QI monitoring and reporting.
Reports results of QI activities to oversight committees and senior staff and Board as indicated.
Coordinates and collaborates with administration in providing comprehensive services to participants.
Participates in Interdisciplinary Team (IDT), when necessary, serves on and chairs committees.
Coordinate and collaborate with administration to ensure comprehensive services are provided to participants.
As a member of the management team, provide coverage for Directors during scheduled absences or emergent situations.
Performs field homecare visits to supervise staff and to evaluate quality of services and staff competency.
Oversee clinical services provided in housing.
Collaborate with both staff and Housing staff to coordinate services, and to maintain compliance with all regulatory requirements.
Performs surveys and audits as indicated to ensure client satisfaction and quality of services.
Participate in all regulatory surveys as needed.
Other duties as assigned.
REQUIRED
A current Registered Nurse or Physical Therapist license in good standing
Bachelor’s Degree in health care administration, nursing, therapy public health or related human services field or equivalent combination of education and experience.
8+ yrs experience in an administrative capacity in a home care agency or other healthcare experience
3+ yrs supervisory or management experience
A minimum of one year experience working with the frail or elderly
Strong understanding of health care quality outcomes and associated metrics.
Demonstrated experience in the utilization of data analytics to improve quality, increase customer satisfaction and drive down cost.
Current First Aid and BLS certifications are required
prior to hire . Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
PREFERRED
Nursing, Medical and/or Clinical Informatics degree
