Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility. Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline. Collaborate with facility staff, interdisciplinary teams, and family members to support patient care. Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR). Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for. Effective oral and written communication in English Basic proficiency with technology, including electronic health records (EHR). Strong organizational and documentation skills, with attention to regulatory compliance. Ability to provide in-person services at assigned facilities, up to 45 minutes. Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day. Full Vaccination and Booster Status may be required in some facilities. xsgimln
