Exciting opportunity to join a collaborative leadership team as the Interim Director of Case Management & Social Work overseeing hospital-wide case management and social work services across three sites. This leader will guide operational performance, optimize patient flow, and foster a culture of teamwork and accountability.

Highlights:

• Oversight of case management and social work across 3 hospital sites with ~50 FTEs and 3 direct reports
• Reports to hospital leadership and partners closely with interdisciplinary teams
• Lead efforts to optimize staffing, patient flow, and post-acute care coordination, with focus on ER case management and hospital-wide services
• Foster a collaborative, stable team culture with low vacancy rates and strong integration with hospital leadership

Requirements:

• VA or Compact RN Licensure required
• Case Management certification preferred, or must obtain within one year of hire
• Experience in hospital case management and post-acute care coordination required
• Proven leadership skills with ability to assess staffing priorities and realign resources as needed
