In this pivotal leadership role, you will report directly to the Executive Director or Administrator and oversee patient care activities to ensure the delivery of exceptional hospice services. Take charge of clinical operations, coordinate care, and develop your team to uphold the highest standards of patient care and regulatory compliance. Your efforts will directly impact the lives of our patients and their families. As a Hospice Patient Care Director, You Will: Drive and oversee patient care services to ensure high-quality hospice care is delivered. Manage scheduling for visits, addressing reassigned, missed, declined, or rescheduled visits with clinical judgment. Triage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner. Lead Interdisciplinary Group (IDG) meetings, presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes. Serve as a mentor and clinical support to staff, fostering their development and ensuring performance and compliance. Guarantee that all care delivery meets federal, state, and local regulatory standards, as well as our company policies. Support continuous improvement initiatives, quality assessments, and operational efficiencies at your location. About You - What You'll Bring: Current RN license in the state of employment. Graduate of an accredited nursing program. A minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting. Strong understanding of hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC). A deep sense of compassion for terminally ill patients and their families. Proven ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds. Excellent interpersonal, organizational, and leadership skills. Proficiency in clinical documentation and technology systems. Preferred Qualifications (Not Required): Bachelor of Science in Nursing (BSN). Previous leadership, team management, or supervisory experience in hospice or home health. Familiarity with EMR systems and telehealth platforms.
