This role is ideal for a licensed Registered Nurse (RN) with strong leadership experience in home health, hospice, or clinical operations management. Key Responsibilities: Direct the day-to-day operations of the home health branch, ensuring compliance with all applicable federal, state, and accreditation standards (CMS, Medicare, Joint Commission, etc.) Oversee the coordination of patient admissions and discharge planning to ensure a seamless patient experience Provide clinical oversight and leadership to interdisciplinary home health staff, including RN Case Managers, LPNs, therapists (PT, OT, ST), aides, and administrative personnel Interview, hire, train, supervise, and evaluate staff while fostering a supportive and collaborative team culture Promote best practices in skilled nursing, chronic disease management, and patient-centered home care Monitor and improve operational efficiency, service delivery, and patient satisfaction Lead performance management efforts, including staff evaluations, scheduling, workload distribution, and addressing employee concerns Ensure continuous survey readiness and participate in quality assurance and performance improvement (QAPI) initiatives Manage branch financial performance, including budgeting, payroll, supply costs, and referral development Build and maintain strong relationships with physicians, hospital discharge planners, and community referral sources About You

You excel at balancing clinical excellence with operational efficiency, and you’re passionate about empowering your staff to provide the best possible patient experience. Qualifications – What You'll Bring: Current RN license in good standing in the state of practice.

Associate's degree in Nursing or related field.

2+ years of experience as a Home Health Administrator, Director of Operations, or equivalent leadership role.

Strong knowledge of CMS regulations and home health compliance requirements.

Proven leadership, communication, and problem-solving skills.

Preferred Experience (Not Required) Experience as an interim or turnaround leader in home health.

Bachelor's degree in Nursing or related field. Background in managing multi-site or high-volume agencies.

Track record of improving operational metrics in a short time frame.
