Overview
We are seeking an experienced Director of Patient Care Services to lead clinical operations for our Cleveland-based home health agency. This role is responsible for ensuring exceptional patient outcomes, maintaining compliance with regulatory standards, and driving operational and financial performance. The ideal candidate is a strategic leader with a passion for delivering high-quality, compassionate care in a home-based setting.


Key Responsibilities

Leadership & Strategy

  • Direct and oversee all patient care services, ensuring alignment with organizational goals.
  • Develop and implement care programs to enhance quality, expand services, and improve patient satisfaction.
  • Build and maintain relationships with community organizations, referral sources, and healthcare partners.

Operations Management

  • Own the P&L for the home health service line, monitoring budget performance and implementing corrective actions as needed.
  • Oversee the care delivery process, ensuring efficiency, timeliness, and seamless coordination.
  • Track KPIs, analyze data, and leverage technology (EHR systems) to optimize performance.

Compliance & Quality

  • Ensure adherence to Medicare, Medicaid, and accreditation standards.
  • Conduct quality reviews and implement evidence-based practices to improve patient outcomes.
  • Identify and mitigate risks to patients and staff.

Team Development

  • Recruit, train, and mentor clinical staff, fostering a culture of accountability and professional growth.
  • Provide ongoing feedback, recognize achievements, and support career development.
  • Promote collaboration across interdisciplinary teams.

Patient & Family Engagement

  • Ensure compassionate, patient-centered care tailored to individual needs.
  • Educate and guide patients and families on care plans and health goals.
  • Represent the organization at community events and outreach programs.


Qualifications

  • Bachelor's degree in Nursing (BSN), Doctor of Physical Therapy (DPT), Healthcare Administration, or related field required; Master's degree or advanced certification preferred.
  • Minimum 5 years of home health experience, including at least 2 years in a leadership role managing clinical operations.
  • In-depth knowledge of OASIS, Medicare/Medicaid regulations, and accreditation requirements.
  • Proven ability to lead interdisciplinary teams and drive operational and clinical excellence.
  • Strong organizational, communication, and problem-solving skills.
  • Proficiency with EHR systems (e.g., HomeCare HomeBase).


Work Environment

  • Office-based with frequent field visits to oversee patient care, support staff, and engage referral sources.
  • Fast-paced, patient-focused setting requiring adaptability and decisive leadership.
  • Local travel required within the service area.
