Overview
We are seeking an experienced Director of Patient Care Services to lead clinical operations for our Cleveland-based home health agency. This role is responsible for ensuring exceptional patient outcomes, maintaining compliance with regulatory standards, and driving operational and financial performance. The ideal candidate is a strategic leader with a passion for delivering high-quality, compassionate care in a home-based setting.
Key Responsibilities
Leadership & Strategy
- Direct and oversee all patient care services, ensuring alignment with organizational goals.
- Develop and implement care programs to enhance quality, expand services, and improve patient satisfaction.
- Build and maintain relationships with community organizations, referral sources, and healthcare partners.
Operations Management
- Own the P&L for the home health service line, monitoring budget performance and implementing corrective actions as needed.
- Oversee the care delivery process, ensuring efficiency, timeliness, and seamless coordination.
- Track KPIs, analyze data, and leverage technology (EHR systems) to optimize performance.
Compliance & Quality
- Ensure adherence to Medicare, Medicaid, and accreditation standards.
- Conduct quality reviews and implement evidence-based practices to improve patient outcomes.
- Identify and mitigate risks to patients and staff.
Team Development
- Recruit, train, and mentor clinical staff, fostering a culture of accountability and professional growth.
- Provide ongoing feedback, recognize achievements, and support career development.
- Promote collaboration across interdisciplinary teams.
Patient & Family Engagement
- Ensure compassionate, patient-centered care tailored to individual needs.
- Educate and guide patients and families on care plans and health goals.
- Represent the organization at community events and outreach programs.
Qualifications
- Bachelor's degree in Nursing (BSN), Doctor of Physical Therapy (DPT), Healthcare Administration, or related field required; Master's degree or advanced certification preferred.
- Minimum 5 years of home health experience, including at least 2 years in a leadership role managing clinical operations.
- In-depth knowledge of OASIS, Medicare/Medicaid regulations, and accreditation requirements.
- Proven ability to lead interdisciplinary teams and drive operational and clinical excellence.
- Strong organizational, communication, and problem-solving skills.
- Proficiency with EHR systems (e.g., HomeCare HomeBase).
Work Environment
- Office-based with frequent field visits to oversee patient care, support staff, and engage referral sources.
- Fast-paced, patient-focused setting requiring adaptability and decisive leadership.
