Sarah S. Brayton Nursing Center Rehabilitation is a short-term rehabilitation and long-term care center, seeking a MA licensed Director of Social Service to join our team. The right candidate must possess a Massachusetts license as an LSW and have proven long term care experience. LTC experience is required . Duties and responsibilities include and but are not limited to: psychosocial assessments
completion of the MDS- mood, behavior and cognition sections
quarterly assessments
attend and provide input at care plan meetings
follow-up on complaints and grievances
provide ongoing support to residents and their families.
