Director of Primary Care Northeast Oregon Location: Pendleton & La Grande, Oregon Organization: Praxis Health About the Role Praxis Health, the Pacific Northwests largest independent medical group, is seeking a Director of Primary Care to lead our Northeast Oregon region (Pendleton and La Grande). This is a unique opportunity to take full operational and financial responsibility for a growing primary care market while shaping the future of care delivery in rural Oregon. As Director, you will: Lead multiple primary care and urgent care clinics with direct oversight of managers, providers, and care coordination teams. Drive profit & loss performance with a focus on growth, cost containment, and quality. Recruit, engage, and retain top managers, staff, and providers in a competitive market. Champion the transition to value-based care, quality-based payment models, and Patient-Centered Primary Care Home (PCPCH) standards. Represent Praxis Health with community partners, state agencies, and local stakeholders. Inspire a culture of continuous improvement through analytics, technology, and innovation. This is more than an operations role, it's an entrepreneurial leadership opportunity with visibility and impact. The Ideal Candidate Were looking for a growth-oriented leader with: Education: Bachelors in Healthcare Administration, Business Administration, or related field (Masters preferred). Experience: Minimum of 3 years managing clinical teams; ideally 510 years in ambulatory or primary care leadership with proven P&L responsibility. Skills: Strong financial acumen and operational expertise. Track record of recruiting and building high-performing clinical teams. Ability to leverage analytics, predictive models, and dashboards to drive decisions. Knowledge of Rural Health Clinic (RHC) compliance and value-based care models.
