Job Details Job Location Albany, NY Position Type Full-Time Salary Range $92000.00 - $92000.00 Salary/year Travel Percentage Up to 25% Job Shift Day Join Our Mission The Director of Nursing is responsible for overseeing the medical needs of youth within Foster Care including but not limited to conducting assessments, treatment planning, medical case management/care coordination, health education/promotion and youth / family supports and services. The Director of Nursing is also responsible for supervision of LPNs as well as creating and maintaining systems that assure the department is meeting all guidelines including, the 29i license, OCFS regulations and COA standards.
Description Job Responsibilities
Medical Case Management and documentation for medical and behavioral health appointments and updates. Direct supervision, monitoring and conducting:
Conduct assessments, including HIV risk screenings, intake documentation, obtain family health history, and triage activities.
Routine screening for developmental health and coordinate physical body checks for abuse.
Follow up after medical appointments, community provider visits, urgent/emergency care or hospitalization.
Establish and maintain treatment plans with contribution to and participation by the LPNs. Utilizing historical & current available information in collaboration with the multidisciplinary team in maintaining the plan of care.
Routine management and training regarding health education.
Coach staff, youth, and families on medication administration and management including reconciliation.
Assist Psychiatric Mental Health Nurse Practitioner:
Complete Prior Authorizations
Call pharmacies for medication verification
Call/ fax labs to schedule lab draws and get results
Call providers for follow up and records release
Educate or share resources to staff, youth, and families on how to maintain medical supplies and equipment.
Practicing proper infection control.
Drives the strategy, direction, and execution of the 29i Nursing program internally and externally through relationship building, project management and program development.
Monitors compliance with service delivery and documentation as indicated in the 29i Regulations for service delivery.
Provision of supervision to LPN's.
Provide on-call administrative oversight when necessary.
Ensures adequate nursing staff coverage hours.
Ensures compliance with state and agency law and regulations.
Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.
Represents TFY's Nursing services in the community and throughout state.
Serves on various agency and /or external committees; acts as liaison to community networks including schools, mental health programs, OCFS and Managed Care Organizations.
Perform other duties as assigned.
Job Requirements
Current New York State Registered Nurse license and CPR/First Aid certification.
Minimum 1 - 3 years of experience. Experience with at-risk youth preferred.
Minimum 1 year of supervisory experience.
Flexible schedule and ability to provide weekend, holiday, and on-call coverage.
Must have current, valid driver's license per Agency guidelines. Travel required.
Ability to work with a culturally and economically diverse population.
Salary
$92,000 annually
