Sonoma Post Acute is hiring a Director of Nursing (DON) to lead our clinical team in providing exceptional care to our residents.
We're seeking a confident, compassionate RN leader with strong post-acute experience and a passion for mentorship, quality care, and team culture.

Responsibilities:
* Provide clinical oversight and leadership for the nursing department
* Ensure high-quality, person-centered care and regulatory compliance
* Supervise and support nursing staff, promoting education and growth
* Develop and monitor individualized care plans
* Work collaboratively with interdisciplinary teams and physicians
* Participate in quality assurance and improvement initiatives
* Serve as a key point of contact for residents and their families
Requirements:
* Current RN license in California
* Previous DON or ADON experience in a skilled nursing facility required
* Strong knowledge of Title 22, OBRA, and CDPH regulations
* Excellent leadership, communication, and organizational skills
* Ability to motivate and mentor a team in a fast-paced environment
* Passion for elder care and dedication to resident well-being
