Lead a dedicated care team consisting of care promoters, facilitators, and coordinators to support patients who can visit the office. Collaborate with transitional care teams for patients unable to visit the office due to hospitalization, skilled nursing facility stay, or other circumstances. Lead weekly clinical meetings like Super Huddles (SH) and Transforming Care Meetings (TCM), and support center operations as needed. Actively recruit patients and interview potential PCP candidates for the company. Mentor and coach PCPs to ensure outstanding clinical outcomes. Partner with Center Operations Directors and General Managers to enhance center financial performance and grow patient membership.
