Overview:
We are excited to invite an experienced Director of Hospice Medical Services to join our dedicated team. In this role, you will:
Act as a vital medical resource for the hospice team, patients, families, and attending physicians focusing on effective pain and symptom management.
Engage actively in interdisciplinary group (IDG) meetings to ensure patient medical needs are thoroughly addressed.
Oversee and guide the development of individualized care plans.
Collaborate with patients' attending physicians to refine and update the plan of care.
Assess and certify patients for hospice admission based on medical criteria, signing necessary documents to validate the admission.
Work closely with the Admission Team, providing essential guidance and support.
Evaluate clinical information leading up to recertifications, delivering written recertifications of terminal illness for ongoing benefit periods.
About You:
Possess a Doctoral Degree in medicine (M.D. or D.O.) and maintain a current, unrestricted medical license in your state of practice.
Demonstrate exceptional leadership skills with a strong commitment to enhancing the care of hospice and palliative patients.
Hold a respected reputation in the community as an authority in hospice and palliative medicine.
Exhibit proficiency in collaborating effectively with an interdisciplinary team.
Have experience working within a distributed medical staff or similar structure.
Maintain a current and valid National Provider Identifier (NPI).
What We Offer:
