Our team is growing, and we’re looking for a strategic, organized, and motivated Director of Coordination to lead and oversee all aspects of client care coordination, staffing, and operational flow.
Position Summary: The Director of Coordination plays a critical leadership role in ensuring that our clients receive timely, consistent, and high-quality home care services. This person will manage the coordination team, oversee scheduling and caregiver assignments, and serve as a key liaison between field staff, clients, families, and clinical leadership. The ideal candidate is a detail-oriented problem solver with strong leadership skills and experience in home health or healthcare operations.
Key Responsibilities:
Lead and manage the coordination team to ensure efficient and accurate scheduling of home health aides and nurses.
Oversee daily operations related to staffing, visit assignments, shift coverage, and care transitions.
Ensure compliance with all state and federal regulations, including DOH and agency protocols.
Work closely with the Director of Nursing and clinical staff to coordinate client care plans.
Handle escalated issues related to caregiver coverage, client satisfaction, and staff performance.
Use scheduling software and reporting tools to monitor productivity and optimize staffing.
Develop and implement systems to streamline communication between departments.
Train, mentor, and evaluate coordination staff to ensure operational excellence.
Qualifications:
Bachelor’s degree in healthcare administration, business, or a related field preferred.
3–5 years of experience in home health or healthcare coordination (supervisory or management experience strongly preferred).
Strong understanding of home care regulations, staffing requirements, and agency operations.
Excellent organizational, leadership, and communication skills.
Proficiency in scheduling platforms (e.g., HHAeXchange, ClearCare, AlayaCare, etc.) and Microsoft Office Suite.
Ability to thrive in a fast-paced, deadline-driven environment with a high level of accountability.
