Description: GENERAL SUMMARY OF DUTIES: The Director of Clinical Services will play a critical function in supporting and driving the execution of highly complex, high impact clinical programs that transform health care delivery. The Director of Clinical Services will work hands-on with providers, executive leadership team, vendors, and other clinical teams and partners to design, prioritize, and implement innovative clinical initiatives; in addition to, managing the performance of clinical initiatives currently in place.

ROLE AND RESPONSIBILITIES: Facilitate and manage a portfolio of various clinical programs at different stages of development and implementation. Establish collaborative, longitudinal relationships with the operations team, medical leaders, external providers, and clinical teams. Flex to produce content in short sprints when appropriate, while also managing longer term programs and projects - working collaboratively with Executive Director of Value Based Care and other leaders in the development of clinical program presentations, formulating hypotheses, and identify operational barriers/risk impacting clinical program performance. Responsible for the creation, implementation and management of clinical programs across a broad spectrum of business opportunities in support of the company’s vision to maintain and develop initiative to support a high performing care model. Ensures the effectiveness of clinical programs through monitoring, measurement, and implementation of performance improvement strategies as indicated Remain current on healthcare industry trends and value-based care models. Works collaboratively with the Population Health Manager and leadership with PHC Research and other teams as assigned on daily clinical operations, daily problem-solving of clinical and operational issues, and the development and revision of policies and procedures. Relationship building with external provider partners including coordinating performance reviews for defined preferred providers. Acts as liaison between PHC and external providers including but not limited to hospital, skilled nursing facilities, hospice and palliative care, and home health. Collaborate on value base opportunities, performance outcomes, and aligning clinical programs. Monitors feedback from physicians. Continuously assesses current clinic flows, processes, and procedures, identifies areas of improvement, and develops and implements best practices and appropriate changes to improve outcomes. Participate in continuing professional growth through attendance at workshops and professional in-services and through individual research and reading, to include communication skills. Attend and participate in organized functions of PHC and perform administrative functions as necessary. Demonstrate personal responsibility and respect for patients, patients’ families, and co-workers in professional appearance. Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams. Others duties as assigned. Requirements: QUALIFICATIONS RN with 3+ years’ experience in managing Clinical Program Development, Quality Improvement and/or Population Health programs in a payor, provider, or population health organization BSN preferred 5+ years of clinical experience Care Management experience preferred Current, active, and unrestricted certification/license Experience working on clinical models in risk-based financial arrangements Experience working with health IT applications and data Demonstrate ability to work as part of the team Experience leading work teams Planning and organizational skills Understanding of program or project management processes Excellent communication skills KNOWLEDGE AND SKILL REQUIREMENTS: Demonstrates the ability to effectively prioritize and execute tasks in a timely manner while eliciting cooperation from a variety of sources, including upper management, peers, and other team members. Ability to work successfully in a fast-paced, stressful environment. ENVIRONMENTAL/WORKING CONDITIONS: Talking, hearing, repetitive motions and close visual acuity associated with a normal office environment. Occasional exposure to clinical / medical office setting. PHYSICAL/MENTAL DEMANDS: Standing, sitting for long periods while doing computer input and making phone calls. Must have ability to bend, stoop, twist and work with computer equipment. May require work during after hours
