The Client Care Coordinator position's main responsibility is client engagement. The responsibilities are to bring in new clients and provide an in-home visit to determine their needs while BHCG is going into the home for services. Once the home visit is complete, you will enter the information into Swyftops and Therap (if a state client) and work with the Director of Operations to match a caregiver to the client for their scheduled needs.
The information should be put into a daily routine and emailed to the caregivers going in with the client.
The Client Care Coordinator will also be responsible for setting up routine visits to go in with clients at the beginning or end of their shifts to continue engagement with the clients. We are going to speak with the clients to see how things are going, and if any changes would assist them, along with getting positive info to be able to provide to caregivers. During this visit, we will also be speaking with the caregiver who is in with the client at that time to see how they are doing. Sometimes follow-up calls may need to be conducted to get the full info to make corrective measures. During these visits, you will also be correcting anything you can on the spot and providing suggestions to correct any dissatisfaction in the home. For these routine visits, you will keep a log of who you visited and any additional notes on what was discussed. You will also be asking the client to leave us a google review if they are satisfied with the services we are providing. If they decline because they don’t do internet, offer to help them leave one.
You will also be working with the Director of Operations for scheduling and interviewing, along with any day-to-day tasks that come up that the Director of Operations or Executive Director asks.
There will also be a 24/7 on-call rotation. As of right now, the rotation will be every 3 rd week. You will be expected to handle the calls coming in. If you have questions, you will reach out to the Executive Director with those questions. When the executive director is on their on-call week you will be expected to be available every other week the executive director has the phone for emergency shift coverage positions.
