Westview Nursing & Rehabilitation - West View Nursing & Rehabilitation Center is a 120-bed facility located in a quiet community in West Warwick.  We are looking for a full time Admissions Coordinator to join our team.
Duties & Responsibilites
*Conducts preadmission interviews with the responsible parties and potential residents.  Evaluates the individual being referred and reviews with the Director of Nursing or designee to determine ability of staff to care for the individual.
*Arranges tours of the facility for potential residents and their families and obtains completed applications.
*Obtains currentinformation on funding, insurance, legal documents as well as advance directives, resident wishes, etc.  insurance authorizations, prescreens medication costs.  Assists in gaining required equipment for resident prior and or upon admission.
*Coordinatesthe admission of new residents and readmission of hospitalized residents.  This coordination includes review of resident needs with the nursing staff on the resident's unit, distribution of appropriate paperwork.
*Obtains appropriate consents and acknowledgements from residents/responsible party either on or before the day of admission, if possible or post admission.
*Visits other agencies, professional organizations, and facilities to gather information that may be helpful to the facility and to relay that information to the appropriate managers.
*Coordinates and completes specific mailings containing information on our facility and its services to interest groups for the purpose of enhancing referral activity.
*Develops collaboratively with Eden senior leadership and updates brochures, public information handouts, and other written materials for distribution to the community.
*Coordinates public speaking engagements to the community when applicable.
*Participates in community-sponsored interest groups and working committees to keep abreast of current information and to participate in appropriate efforts to promote change.
*Conducts tours to prospective admissions.
*Completes community needs assessments as needed.
*Completes special projects as requested by Administration.
*Contributes to the annual marketing plan and updates as appropriate.
*Completes quarterly and annual progress reports.
Qualifications:
Associates degree, at a minimum, or experience within a healthcare setting.
