This position supports our mission by ensuring care coordination services are delivered to enhance our operations and efficiency to drive performance in value-based contracts. This position will oversee the Care Coordination team on a National Level. The position will have direct reports and will be responsible for this team's performance. Primary responsibility will focus on oversight of key performance measures to support Revere Medical’s care coordination activities. The ability to work in a cross-functional role across and within Revere Medical’s performance and business teams is a primary responsibility.

Operating Responsibilities

Care Coordination Activities

  • Ensure all care coordination services are delivered efficiently and effectively in compliance with clinical guidelines, payer requirements, and contractual obligations.
  • Develop and monitor departmental KPIs, dashboards, and reporting mechanisms to track performance and outcomes.
  • Oversee transitions of care, population health initiatives, and other care coordination activities.
  • Collaborate with IT, clinical, and administrative teams to optimize workflows, documentation, and systems performance (e.g., EMR/EHR, case management platforms)
  • Proactively identifies, and then efficiently and competently manages challenges that arise across the Care Coordination team.
  • Evaluates deliverables prepared by the team to make sure the work meets requirements and maintains a high level of quality.
  • Identification of processes and systems that can be refined to be more efficient and proposition of changes to resolve those inefficiencies including formulating and recommending policies and programs
  • Monitor and evaluate compliance with state, federal, and payer-specific regulations, including CMS, Medicaid, and other managed care standards
  • Serve as the primary point of contact for contract performance and reporting requirements related to care coordination services.

Staff Management

  • Directly supervise care coordination team members across clinical and non-clinical roles.
  • Develop and implement staffing plans, training, and strategies to ensure an engaged, high performing team.
  • Conduct regular performance evaluations and coaching to support staff development and accountability.
  • Carrying out and attaining the mission and the goals of the Care Coordination team
  • Cultivates a motivated employee environment of trust, teamwork, accountability, self-confidence, and business ownership.

Specialized Knowledge and Skills

  • Ability to demonstrate leadership skills within a team environment, take control of situations when appropriate and achieve results, and take initiative in representing issues and potential solutions to leadership.
  • Strong knowledge of care coordination best practices, utilization management, and patient-centered care models.
  • Ability to comprehend problems, arrive at solutions, and communicate those effectively to stakeholders.
  • Data-driven mindset with the ability to analyze trends, develop action plans, and drive measurable results.
  • Advanced organizational, problem-solving, and communication skills.
  • Proficiency in care coordination platforms and Microsoft Office Suite

Education

  • Bachelors Degree in a health care field required; Master’s degree preferred.
  • Current licensure as a Registered Nurse, Licensed Social Worker or relevant licensure
  • 5-7 years of healthcare leadership experience
