As the Clinical Associate Medical Director, you will oversee and mentor primary care providers (PCPs) within your assigned center. Your role is crucial in maximizing our clinical model, enhancing quality, improving outcomes, and ensuring patient and clinician satisfaction. You will combine leadership responsibilities with direct patient care, adjusting your focus based on the needs of your team and center.
Key Responsibilities:
Deliver comprehensive care for patients with both acute and chronic health conditions.
Take responsibility for patient outcomes while collaborating with specialists as needed.
Engage with hospitalists for patient care during hospital stays.
Conduct assessments, diagnoses, and treatment plans for complex health needs.
Lead your team, including a medical assistant, care facilitator, and care coordinator.
Coordinate care for patients unable to visit the office, working with transitional care teams and available resources.
Facilitate weekly Super Huddles and Transforming Care Meetings.
Support the overall management of your center, including patient outreach and recruitment.
Mentor and coach PCPs to deliver exceptional care outcomes.
Collaborate with operations leaders to improve center performance and membership growth.
Perform additional duties as assigned by management.
Qualifications:
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
Current, active medical license in your state of employment.
At least 1 year of clinical experience in a geriatric, adult, or family practice setting preferred.
Completion of ChenMed training, including Masterful Conversations.
Board certification in your specialty preferred; Board eligibility is required.
Current DEA number for controlled substances.
BLS certification from an accredited provider required within the first 90 days of employment.
