Licensure/Certification Qualifications: Job Title: Associate Medical Director, Population Health Department: Population Health/Clinical Affairs Reports to: Chief Medical Officer (CMO) of Population Health Employment Type: Part time (0.4 FTE) Remote Position Summary: The Associate Medical Director of Population Health supports the strategic and operational clinical initiatives that support improved health outcomes, quality of care, and cost efficiency across a defined population. This physician leader collaborates with population health leadership, clinical teams, care management, analytics, and community partners to develop, implement, and monitor population health programs. The role combines clinical expertise, data-driven decision-making, and strategic vision to align care delivery with value-based care models. Key Responsibilities: Provide clinical leadership in the design and execution of population health strategies. Work with care management team on clinical case reviews. Partner with analytics teams to identify care gaps, high-risk populations, and opportunities for intervention. Oversee and support care management, care coordination, and chronic disease management programs. Collaborate with physicians, advanced practice providers, and multidisciplinary teams to drive clinical engagement and adherence to evidence-based guidelines. Support quality improvement initiatives focused on reducing hospitalizations, ED utilization, and total cost of care. Work with population health team on appropriateness review of external referrals. Evaluate performance metrics (e.g., HEDIS, STAR, CMS ACO, NCQA) and develop action plans for improvement where needed. Ensure compliance with regulatory standards and value-based payment program requirements (e.g., Medicare Shared Savings Program, Medicaid and Commercial Risk Contracts). Foster a culture of health equity and patient-centered care. Support the development and implementation of digital health, remote patient monitoring, and other innovative care delivery models. Qualifications: MD or DO with active medical license; board certification required. Minimum 5 years of clinical practice experience. 3+ years of leadership experience in healthcare environment Experience working with healthcare analytics, quality metrics, and performance-based programs. Strong communication, collaboration, and leadership skills. Strategic mindset with a focus on innovation, outcomes, and patient-centered care. Ability to function independently and deal with multiple simultaneous projects. Ability to recognize personal strengths and areas for improvement and develop goals for professional growth and achievement. Ability to demonstrate a commitment to quality and excellence.
