POSITION SUMMARY:

As a cause-driven leader and integral member of the Community Wellbeing team, the Associate Executive Director, Community Wellbeing, will support the Executive Director in executing the organization’s comprehensive community health strategies. This leadership role is responsible for overseeing and guiding the Y’s Community Health Worker initiatives, with a strong emphasis on advancing health equity, increasing access to care, and addressing the social determinants of health in underserved communities.

The AED, Community Wellbeing will provide day-to-day supervision, support, and professional development for a team of Community Health Workers embedded across multiple Y family centers and community sites. This individual will ensure that the CHW team is equipped to build trust with community members, conduct effective outreach and assessments, connect individuals to vital resources, and support chronic disease prevention and self-management programs.

In alignment with the Y’s Health Equity Strategy, the AED, Community Wellbeing will also play a key role in developing and refining workflows, evaluating impact, and sharing outcomes to demonstrate effectiveness and secure sustained investment. This includes supporting data collection and reporting, ensuring compliance with grant requirements, and identifying areas for quality improvement.

Additionally, the AED, Community Wellbeing will assist the Executive Director in cultivating and managing partnerships with hospitals, health systems, academic institutions, government agencies, and community-based organizations. These collaborations are critical for expanding access to services, co-designing innovative health initiatives, and improving long-term health outcomes for the populations we serve.

All Y associates agree to abide by The Code of Conduct with Youth that sets clear expectations for associates and volunteers to ensure a safe and respectful environment for young people. It emphasizes treating youth with respect and fairness, avoiding inappropriate physical or verbal interactions, and adhering to established standards of affection. The policy strictly prohibits any form of abuse, bullying, or harassment and mandates the reporting of suspected mistreatment. Additionally, associates and volunteers must complete training on child abuse prevention and comply with legal reporting requirements to safeguard youth.
