As our Associate Director of Clinical Services, you will lead a team of primary care providers (PCPs) in a dynamic environment focused on improving clinical quality and patient satisfaction. This role allows you to maintain a portion of your clinical practice while enhancing leadership capabilities within our rapidly expanding organization. Key Responsibilities: Provide comprehensive care for patients with acute and chronic illnesses, especially in older adults. Manage patient care outcomes and collaborate with specialists effectively. Engage with hospitalist teams for seamless care coordination. Lead a dedicated care team, overseeing care facilitators and coordinators. Support home-based care for patients unable to visit our centers, ensuring all-team coordination. Conduct weekly Super Huddles and Transforming Care Meetings to enhance team performance. Assist in center management, contributing to patient recruitment and provider onboarding. Mentor and coach PCPs to excel in clinical outcomes, emphasizing team collaboration. Collaborate with operational leaders to ensure financial success and growth in membership. Qualifications: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current, active medical licensure in your state of employment. Preferably 1+ years of clinical experience in geriatrics, family medicine, or an adult practice setting. Completion of ChenMed training programs, showcasing our commitment to professional development. Board certification in relevant specialties is preferred; Board eligibility is required. DEA registration for controlled substances is necessary. Basic Life Support (BLS) certification is needed within the first 90 days of employment.
