We are seeking an innovative leader to direct and implement a system of care supports for Medicare-eligible adults with complex social needs in Illinois. This position is crucial in building and sustaining a coordinated network of non-clinical services, including peer support, community health workers (CHWs), and housing navigation, aimed at improving quality of life, independence, and health equity. The Associate Director will play an instrumental role in integrating care management into a person-centered model that addresses social determinants of health through trauma-informed and culturally responsive approaches. You'll be responsible for: Leading the development of a care management model that utilizes extenders such as CHWs and housing advocates. Collaborating with internal teams and external partners for seamless integration of non-clinical supports into care planning. Building and maintaining partnerships with community organizations, housing providers, and public agencies. Promoting culturally responsive and person-centered approaches throughout all care management activities. Monitoring program performance and utilizing data for continuous improvement. Developing policies and procedures supporting high-quality service delivery. Supporting training for care management extenders, including CHWs and peer specialists. Participating in strategic planning to advance health equity and reduce disparities. Assisting in resolving individual member issues related to social needs such as housing and food insecurity. Representing the care management program in collaborative initiatives and community forums. Required Qualifications: Must reside in Illinois (Hybrid Home position with up to 35% travel). Bachelor's degree in Social Work, Public Health, Human Services, or a related field. At least 6 years of professional experience in Medicare or community-based care, including 2 years in a management role. A strong understanding of care management models is essential. Preferred Qualifications: Advanced degree in a related field and 3+ years of experience working with social determinants of health.
