The Associate Clinical Director plays a vital role in supervising and training primary care providers (PCPs) in our centers. This leadership position focuses on enhancing clinical quality and patient satisfaction while also involving direct patient care. You will be a key driver of our core model execution and oversee various clinical operations. Key Responsibilities: Provide exceptional care to patients with acute and chronic illnesses, particularly in older adults. Take full accountability for patient outcomes, coordinating closely with specialists when necessary. Maintain regular communication with hospitalists regarding patient care during hospital visits. Lead a care team and ensure effective management of patients in various settings, including homebound and long-term care facilities. Conduct weekly Super Huddles and Transforming Care Meetings to foster team collaboration and improve patient care. Support clinical leadership, engage in patient recruitment, and mentor PCPs to achieve outstanding clinical outcomes. Partner with center management to drive improvement in center financial performance and enhance patient membership. Fulfill additional responsibilities as assigned to meet organizational needs. Qualifications: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty required. Active medical license in the state of employment is essential. A minimum of 1 year of clinical experience in geriatric or family practice settings is preferred. Board certification in Internal Medicine, Family Medicine, or Geriatrics is preferred, with board eligibility being required. Must maintain necessary certifications, including BLS within the first 90 days of employment, and possess a current DEA number for controlled substances.
