The Assistant Director of Clinical Operations will play a crucial role in supervising and training primary care providers (PCPs) within our center. This role is vital in optimizing our core model execution, improving clinical quality, enhancing outcomes, and ensuring high clinician and patient satisfaction. Alongside overseeing the clinical outcomes of the center, you will also be involved in direct patient care as a PCP, with your time divided between clinical and leadership responsibilities. Key Responsibilities: Provide exemplary care for patients with both acute and chronic conditions, especially in an older adult population. Take full accountability for patient care outcomes, collaborating with specialists as needed while remaining actively engaged in their detailed care. Engage with hospitalists for patients admitted to hospitals, fostering a collaborative approach to patient management. Conduct assessments, diagnoses, treatments, health education, and care coordination for patients with complex chronic health issues. Lead your care team, which includes care promoters and coordinators, ensuring optimal patient care in the office. For patients unable to visit the office, collaborate with transitional care teams, case managers, and acute care staff to provide comprehensive support. Lead weekly care meetings, supporting center clinical leadership as needed to achieve optimal outcomes. Participate in the management of the center, providing coverage for absent providers and engaging in patient recruitment initiatives. Mentor and coach PCPs to ensure top-tier clinical outcomes, observing and enhancing daily huddles. Engage in regular leadership activities with PCPs to foster a cohesive clinical culture. Collaborate with operations leadership to enhance financial performance and increase membership at the center. Perform additional duties as assigned to support organizational goals. Qualifications: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required. Current, active medical license in the state of employment. A minimum of 1-year clinical experience in a geriatric, adult, or family practice setting is preferred. Completion of Chen Medical training is a plus, including Masterful Conversations and meeting facilitation. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related specialty, or board eligibility is required. Current DEA number for controlled substances (Schedule II-V) is necessary. Basic Life Support (BLS) certification required within the first 90 days of employment.
