Position Purpose:
The primary responsibility of your job position is in the management and oversight of the day-to-day functions, including the supervision of health and personal care services, of the Assisted Living in accordance with current federal, state, and local standards, guidelines, and regulations that govern Assisted Living to ensure that the highest degree of quality of care is maintained at all times.
Essential Job Functions:

Organizational Responsibilities:
· Adhere to, support and promote St. Francis Health Services’ Mission and Core Values, philosophy, goals and objectives while conducting company business.
· Maintain confidentiality of company and individual receiving services’ related information at all times.
· Serve on, participate in, and attend various committees as appointed by the Administrator.
· Implement all policies and procedures among staff.
· Monitor standards of care, service, and productivity, ensuring a high quality of life for all individuals receiving services.
· Provide prompt attention to any deficiency notices or other situations requiring corrective action. Counsel staff appropriately and timely concerning areas of deficiency as well as areas of excellence.
· Establish informational systems which aid employees to fully understand the Assisted Living’s (AL) mission, philosophy, and operations.
· Make routine inspections of the AL to assure that established policies and procedures are being implemented and followed. Participate in the organizational development of and updates to policies and procedures.
· Coordinate and manage daily work load, staffing as indicated in the Staffing Plan. Assure appropriate assignments for all services.
· Participate in short and long-range planning, and implement specific measures for growth through education of physicians and other community and referral sources.
· Stay current in health related services and management issues.
· Assure service satisfaction through individuals receiving services’ visits and other measures as appropriate. Responsible for investigating and resolving all complaints.
· Direct and/or provide in-services to all staff. Provide adequate opportunity for education and ensure that all staff meets in-service requirements.
· Is available (or similarly qualified alternate is available) at all times during operating hours.
· Work cooperatively with St. Francis Health Services to publicize services and maintain community relations in order to promote the AL.
· Represents the AL in dealings with outside agencies, including government agencies, third party payers, and at various meetings as necessary.
· Review all incident reports, medication errors, adverse medication reactions and infection reports to identify problem areas or negative trends and determine if additional measures or interventions should be taken.
Individual Receiving Services’ Care Responsibilities:
· Does not disclose individual receiving services’ protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
· Ensure that all care is provided in privacy, as appropriate, and to knock after entering an individual receiving services’ residence.

- Assure an individualized service plan is established and implemented for each individual receiving services and that the plan is periodically reviewed and revised as necessary according to the MDH regulations.
- Ensure all personnel involved in providing care are aware of individual receiving services’ service plan.
- Ensure that all personnel refer to the individual receiving services' service plan prior to providing care to the individual receiving services.
- Regular review of service documentation to determine if the service plan is being followed.
- Ensure that health and personal care is administered in accordance with the individual receiving services' wishes, including the implementation of advance directives.
- Participate in the individual receiving services admission process including assessment to determine appropriateness of admission in terms of assisted living service needed, unlicensed personnel competencies, and subsequent admission to AL.
- Participate in individual receiving services’ discharge process including transfer planning and setting up outside service providers when needed.
- Continuously develop, promote, and assure implementation of standards of nursing practice that are consistent with high quality of care.
Individual Responsibilities:
· Report all complaints and grievances, according to company policies and procedures, which are made by the individual receiving services, family, or visitors and make a written/oral report to the Administrator indicating what action(s) were taken to resolve the complaint or grievance.
· Report any communicable or infectious disease that you contract to the Administrator.
