Responsibilities:
Develops a Social history, social assessment, updates progress notes, and care plan with all Social Service programs that identifies problems and/or concerns, goals, and interventions and updates the plan of care according to State and Federal Regulations
Counsels prospective residents/patients and families, as needed, for adequate preparation for admission to the facility.
Asses the social, emotional, psychosocial, spiritual and cultural needs of the residents/patients and ensures that the social service intervention is a part of the residents/patients plan of care and communicates and documents accordingly
Completes MDS, CAAs, PASARR, and social service related assessments according to State and Federal regulations.
Makes sales calls outside the facility to medical, legal and financial professionals, senior organizations, appropriate special interest groups, hospital discharge planners and other community contacts.
Maintains current data base of existing and potential referral sources. Utilizes sales practices and tracking tools.
Monitors inquiry/ referral, admissions and census trends. Prepares appropriate reports/tracking logs, and other reports as required. Monitors all active and inactive inquiries.
Maintains a database of medical contacts and community resources, along with knowledge of industry trends and legislative/regulatory issues and communicates this information to facility staff and community contacts.
Daily contact with area hospitals.
Ten (10) contacts per week with three (3) contacts being considered new business (the volume of calls may be adjusted by the Administrator based on various factors including other job responsibilities and other team member assigned sales/marketing responsibilities).
QUALIFICATIONS:
Licensed Social Worker according to State and Federal Regulations in which the facility operates
Bachelor’s Degree in related field preferred
One to three years of professional experience and/or training; or equivalent combination
Experience in an admission position or have previously held similar position
Previous health care community relations experience preferred
Knowledge of, or ability to learn, reimbursement program from payer sources.
Knowledge of principles and methods for showing, promoting, and selling products or services. This includes marketing strategy and tactics, product demonstration, sales techniques, and sales control systems.
Knowledge of managed care referral process.
Working knowledge of nursing homes regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid
Excellent communication skills; written and verbal
