SUMMARY
Through comprehensive assessment and evaluation, develops and provides ongoing program of activities designed to meet the interests and physical, mental, and psychosocial well being of each patient.
ESSENTIAL RESPONSIBILITIES include the following. Other duties may be assigned.
Recruits, hires, supervises, conducts orientation, provides training, and retains a sufficient number of qualified staff to carry out the responsibilities of the Activities Department. Endure employee performance meets or exceeds expectations and periodic performance appraisals are conducted on a timely basis. Performs Initial Assessment for each resident to determine level of abilities, past and current interests, adaptations, demographics, cognition, behaviors, and level of communication. When a Change of Condition occurs, reassessment done and notated in chart on new assessment form. Develops appropriate Patient Activity Care Plans by assessing needs, goals, and interventions of each patient with consideration for all levels of interaction and engagement. Supports residents’ needs and desires through 1:1 interaction, small group activity and large group meetings. Records Progress Notes in the clinical record including subjective findings, objective symptoms, observations of behavior, interventions provided to patient, and patient’s response to activity interventions. Reviews staff’s chart entries for completeness and accuracy. Utilizes “biopsychosocial” model to create meaningful activities for individuals by ensuring individualized programs are multifaceted, meet patients’ functional levels, and reflect needs and interests of each patient As part of the Interdisciplinary Team (IDT), attends morning meetings, care plan meetings, and assists in evaluating efficacy of interventions for nursing and physician actions. When applicable, conducts Safe Smoking Assessment, and develops and writes a Care Plan for Injury From Smoking. Develops and utilizes a budget sufficient to maintain equipment and purchase supplies, enabling staff to provide prescribed activity models. Creates, organizes, and manages resident activities calendar and communication tools. Posts monthly activities calendars that reflect scheduling, choices and rights of patients’, offer activities at hours convenient to patients, reflect the cultural and religious interests of resident population, and appeal to all genders and age groups living in the facility. Assists residents and staff in orientation to time and place by strategic use of center-wide bulletin boards and appropriate holiday decorations and reminders. Performs the administrative functions for booking/hiring, scheduling, and paying (if relevant) speakers, presenters, advocates, and clergy to interact with individuals and groups of residents. Coordinates and manages volunteer program in compliance with Mariner policy. Coordinates Resident Council agenda, communication and meetings. Arranges and coordinates off-site activity trips, when available and appropriate. Assists residents and guests to activities with the help of direct care staff. Conducts daily Ambassador Rounds, documents findings, reports findings at morning meeting. Acts as Manager of the Day (MOD) when necessary and write daily report. Fosters a sense of community within the facility. Conducts job responsibilities and completes from and documents in accordance with company policy and state and/or federal regulations. CLINICAL RESPONSIBILITIES
Collaborates with Rehabilitation Department to arrange activities that promote functional independence such as providing range of motion activity plan with and for restorative nursing services. Communicates relevant data to other disciplines such as providing information on fluid and snack intake at activities to nutrition services Conducts in-service training to educate all-staff on activities approaches and models Collaborates with social services personnel in developing intervention strategies for residents with evident psychosocial concerns. Consults with IDT Team on residents who may have behavioral disturbance or symptoms of a mood disorder, making recommendations based on observed and documented interests and needs. Utilizes Quality Indicators/Quality Measures Report to evaluate program efficacy for individuals and overall performance; makes program changes when needed. Assists resident/family and social services with discharge planning, by providing insights on effective activity interventions.
QUALIFICATIONS
Bachelor’s Degree in Recreational Therapy, Occupational Therapy, or human service field preferred or Certification as an Occupational Therapy Assistant, Recreational Therapy Specialist, or Associate Degree in human service field with relevant experience or Certified Activity Director (NCCAP or state specific) Meets all state-specific requirements Proficient in Microsoft Office and using a computer.
SUPERVISORY RESPONSIBILITIES
Supervises activity assistant, volunteers, and others whom they are administratively or professionally responsible.
PHYSICAL DEMANDS AND ENVIRONMENT
Employee is frequently required to stand, walk, use hands and/or fingers, reach with hands and arms, feel, talk, and hear. Employee will occasionally assist to lift and/or transfer patients weighing between 100-250 pounds. Employee is occasionally exposed to blood or other body fluids, and fumes or airborne particles. OTHER REQUIREMENTS
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident’s protected health information. Monitors and oversees facility’s efforts to comply with company policies, procedures and state and federal law.
