: Active Indiana license Active Indiana DEA or easily transferable CSR status Epic experience - Preferred CV must be in Month / Year format Must have license in hand at time of submission, or proof that the license application has been submitted Any gaps greater than 30 days will require an explanation on the CV DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
