Job Summary : Portland Health and Rehabilitation Center is currently seeking a Social Services Director to assist with maintaining the social service program designed to meet the individual physical, mental, and psychosocial needs of the residents. Essential Functions : Work with residents, families, and community support to complete social service history, discharge plans, and development of interdisciplinary care plans designed to meet the bio-psychosocial needs of residents. Assist residents and responsible parties in locating and utilizing financial, legal, mental health, and other community resources. Coordinate and/or provide health and mental health social work services to residents to help attain or maintain the highest practical mental and psychosocial well-being, and assist residents displaying mental or psychosocial difficulties to receive appropriate treatment and services. Provide resource support to staff participating in behavioral interventions for residents. Coordinate safe discharge planning and follow-up services in collaboration with the interdisciplinary team. Education, Licensure, and Experience : Effective communication skills with residents, families, and staff. Preferred: a minimum of a Bachelor's Degree in an accredited program of social work.
