The Social Services Director is responsible for planning, developing, organizing, and directing the social service programs of the nursing home in accordance with federal, state, and local standards and regulations. The goal is to help residents attain and maintain their highest practicable physical, mental, and psychosocial well-being. The ideal candidate will possess a strong background in social work and demonstrate a commitment to enhancing the quality of life for those served.
Responsibilities
Assess the psychosocial needs of residents and develop individualized care plans.
Provide support to residents and families during admission, discharge, or adjustment periods.
Advocate for residents’ rights and assist in resolving grievances or concerns.
Participate in interdisciplinary care plan meetings and contribute to resident assessments (MDS).
Coordinate services with outside agencies (e.g., mental health, hospice, transportation).
Facilitate resident and family councils or support groups.
Ensure proper documentation of social services activities, assessments, and interventions.
Assist in discharge planning and coordination of post-discharge care.
Maintain confidentiality and compliance with HIPAA regulations.
Skills
Bachelor’s degree in Social Work (BSW) or related field required; Master’s degree (MSW) preferred.
Minimum of 1–2 years’ experience in a long-term care or healthcare setting.
Knowledge of Medicare/Medicaid, discharge planning, and psychosocial assessment tools.
Must have knowledge of state and federal guidelines
Excellent management skills and experience in supervising staff and fostering a collaborative team environment.
Strong communication, organization, and interpersonal skills.
Must meet all local health regulations, and pass post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry.
Compassionate and culturally sensitive approach to diverse resident populations.
Must have ability to read, write and follow oral and written directions at a level necessary to accomplish the job.
Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation.
Working Conditions:
Work is performed in a nursing home environment with frequent resident and family contact.
May require occasional evening or weekend hours.
Ability to handle emotionally charged situations with professionalism.
