All American Village Nursing and Rehabilitation Center - Purpose: To provide social services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization.
Qualifications & Essential Requirements:
Master’s degree (MA/MS) in Social Work or a related field. Active state licensure or license eligibility required.
Minimum of 2 years’ experience in social services, including at least 1 year in a supervisory role.
Proficient in English (reading, writing, speaking).
Strong independent decision-making skills.
Ability to communicate tactfully and effectively with residents, families, staff, and outside agencies.
Demonstrated leadership and the ability to work collaboratively.
Physically capable of performing job duties including walking, standing, sitting, climbing stairs, and computer use.
Must be emotionally and physically capable of assisting in emergencies and evacuations.
Exposure Risk: Level II (Moderate Exposure Risk): May involve limited or indirect exposure to blood, body fluids, infectious diseases, and hazardous chemicals.
1. Direct Service & Programming
Conduct intake interviews and assessments.
Assist residents with Advance Directives and guardianship paperwork.
Help residents obtain vital documents (IDs, birth certificates, social security cards, voter registrations).
Participate in care plan development and care conferences.
Facilitate treatment interventions to support resident independence.
Lead skills groups, therapy groups, and individual counseling as assigned.
Act as a liaison with outside agencies (e.g., ORS/DHS, Senior Services, court personnel) to coordinate communication and services.
Assist in discharge planning by connecting residents with job training, employment services, housing, and other transitional resources.
Provide support to other staff when managing resident issues or complex care needs.
Follow up on resident concerns and unresolved issues as needed to ensure continuity of care.
2. Record Keeping
Maintain clear and comprehensive case records per policy.
Monitor documentation to ensure compliance with QI standards.
Participate in developing and implementing OSHA, JCAHO, and IDPH standards.
Maintain current records of resident guardianship and legal designations (POAs, DNRs, etc.).
3. Agency Training and Activities
Attend and participate in in-services and trainings.
Engage in clinical staffing, meetings, and special projects.
Set professional goals and participate in job-related staff development.
4. Miscellaneous Duties
Ensure compliance with Abuse & Neglect policy enforcement.
Maintain HIPAA compliance at all times.
Safeguard passwords and follow computer security protocols.
Support and contribute to facility QI programs, especially in dietary services.
Report findings and recommend corrective actions when needed.
Perform other duties as assigned.
