Location: Live Oak, Florida Facility: Live Oak Healthcare and Rehabilitation Shift: Full-Time | 8:00 AM – 5:00 PM Salary: Up to $74,880/year (commensurate with experience)

Make a Real Impact Every Day Live Oak Healthcare and Rehabilitation is seeking a compassionate, organized, and experienced Social Services Director to join our leadership team. In this vital role, you will oversee our social services programs, support residents and families, and help ensure quality care in alignment with care plans and state/federal guidelines.

You will work closely with our interdisciplinary team (IDT) and administration to ensure our residents receive the emotional, mental, and social support they need to thrive.

Key Responsibilities Include:

Coordinate discharge planning, financial assistance, and community referrals for residents and families.

Ensure residents receive necessary behavioral health care in line with their care plans.

Participate in resident/group council meetings and educate on resident rights and advance directives.

Keep current with regulations and recommend policy/procedure updates.

Provide translation assistance for residents with limited English proficiency.

Maintain a resource library of regulatory and compliance materials.

Collaborate with all departments to enhance quality of life for residents.

Support facility surveys and inspections as needed.

Build partnerships with community agencies to support resident needs.

Qualifications:

Minimum 3 years of supervised social work experience in a healthcare setting.

Strong leadership skills with the ability to supervise and collaborate effectively.

Compassionate, tactful, patient, and adaptable to different cognitive levels.

Up to date with state/national licensing or accrediting requirements.

Committed to professional growth and continuous learning.

Must be able to communicate clearly about resident needs and conditions.

Must not pose a direct threat to the health or safety of others in the workplace.
