Cherry Hill Nursing and Rehabilitation - Social Services Director Location: Cherry Hill Nursing and Rehab – Westland, MI Salary: $70,000 annually

Cherry Hill Nursing and Rehab is seeking an experienced Social Services Director to join our skilled nursing facility in Westland, MI. In this role, you will serve as a liaison and advocate for our residents’ social and emotional needs, while overseeing the planning, organization, and daily operations of the Social Services Department.

Key Responsibilities

Develop comprehensive social assessments and care plans, identifying residents’ psychosocial needs and setting realistic goals with actionable steps.

Participate actively in the pre-admission and admission processes.

Provide compassionate, solution-focused support to residents and their families during times of adjustment, crisis, or special need.

Document residents’ behavioral and psychosocial status accurately and timely.

Coordinate individual and group interventions for residents and families.

Establish and manage a volunteer program tailored to specific social service needs.

Orient new residents and families to the facility, ensuring a smooth transition.

Qualifications

Bachelor’s Degree in Social Work or a related field from an accredited institution.

Knowledge of social services practices, procedures, laws, and regulations applicable to long-term care.

Strong interpersonal and communication skills, with the ability to work independently.
