JOB SUMMARY: A Social Services Director working with our Skilled Nursing facility will counsel and assist residents and families requiring support. This role involves providing individual and group social services programming, promoting residents' potential, and collaborating with physicians and residents in discharge planning. RESPONSIBILITIES: Interview all new residents to assess their psychosocial needs Coordinate admission and discharge planning Counsel residents individually or with family/support groups Refer residents to services such as Dental, Optometry, Audiology, Podiatry, Psychology, Psychiatry, and other community health resources Compile records and prepare reports on theft and loss Coordinate with nursing to meet optical, dental, and audiological needs and arrange transportation Document social service interventions and assessments in residents' charts Attend resident care meetings, rehabilitation, utilization review, quality assurance, and department meetings Complete sections of MDS timely Perform other duties as assigned REQUIREMENTS: Bachelor's Degree in Social Services or a related field Skilled Nursing experience preferred EMPLOYEE BENEFITS: PTO 401(K) Medical/Health Insurance Dental Insurance Vision Insurance Birthday/Holiday Pay Wellness Fund Uniform Fund Longevity Bonuses Quarterly Vacation Drawing Tuition Reimbursement HOW TO APPLY: If you are a passionate healthcare professional looking to make a difference, we encourage you to apply!
