Healthcare Nursing of Venice - Director of Social Services

We are seeking a full-time Director of Social Services to lead and support the social, emotional, and psychological well-being of our residents. This role is essential in ensuring residents reach their fullest potential during their stay and experience a smooth and thoughtful discharge process. The Director of Social Services also oversees the Social Work Department for both long-term and acute care populations.

Director of Social Services Key Responsibilities:

Develop comprehensive social histories, assessments, and individualized care plans that identify each resident’s needs, establish realistic goals, and outline action steps to meet those goals upon admission.

Participate actively in interdisciplinary Care Plan meetings to coordinate holistic resident care.

Maintain timely and accurate documentation of progress notes aligned with each resident’s care plan.

Support residents and families in discharge planning, including placement coordination and transitional care needs.

Director of Social Services Qualifications:

Bachelor’s degree in Social Work or a related field (required).

Working knowledge of computer systems, including email and electronic health records.

Ability to communicate effectively and compassionately with residents, families, team members, and external partners.

Familiarity with state and federal guidelines pertaining to long-term care and social services.
